MDR-TB is the incidence of TB resistant to at least two of the most potent anti-TB drugs, namely rifampin and isoniazid. The main problems that arise in MDR-TB patients are difficult and expensive treatment, high mortality rates and the potential to transmit resistant bacilli to others. The highest MDR-TB cases in South Sumatra were in the city of Palembang. Based on data from the Palembang City Health Office, MDR TB cases in Palembang City from 2015-2020 were 233 cases. The purpose of this study was to analyze the risk factors for the incidence of MDR-TB in Palembang City in 2021. This study uses a case control study design. The number of respondents in this study were 120 people, consisting of 60 cases and 60 controls. Data was collected using a questionnaire with purposive sampling technique. Data were analyzed by univariate, bivariate (using Chi Square test) and multivariate using predictive model logistic regression test. The results of the study showed that there was a significant relationship between duration of treatment (p-value 0.003) and medication adherence (p-value 0.001) with the incidence of MDR-TB in Palembang City. Whereas, variables of age, gender, smoking history and distance to health facilities were not associated with the incidence of MDR TB in Palembang City. The most dominant factor influencing the incidence of MDR TB in Palembang City was medication adherence (AOR 4,621;95%CI=1,879-11,366).Cooperation is needed from various related parties, especially from patients and health workers to prevent the occurrence of MDR TB and the risk of MDR TB transmission. Compliance with taking medication, high motivation to complete TB treatment, disseminating information to the public about TB and MDR TB and conducting active surveillance to detect suspected TB and MDR TB are very important things to prevent the occurrence of MDR TB.
