One of the effects of pesticides on health is a decrease in hemoglobin levels. The purpose of this study was to analyze the relationship between pesticide exposure and hemoglobin levels in horticultural spray farmers in Lingga Julu Village, Simpang Empat District, Karo Regency. This research is a quantitative analytic using cross sectional design with purposive sampling technique as many as 110 pesticide spraying farmers. Data collection was carried out by interviewing and filling out questionnaires and measuring blood samples to determine hemoglobin levels using the Easy Touch GCHB tool. Data are analyzed by univariate, bivariate using the Chi-square test and multivariate using the Logistic Regression model prediction test. The results showed 23.6% of respondents had low hemoglobin levels. The results of multivariate analysis showed that the variable that proved to be a risk factor for reducing hemoglobin levels in horticultural spray farmers was the frequency of spraying (p-value = 0.012; OR = 6.267; 95% Cl = 1,490-26,364), spraying time (p-value = 0.038; OR = 3,566; 95% Cl = 1,071-11,870), years of service (p-value = 0.049; OR = 9.963; 95% Cl = 1,006-98,632), and the number of pesticides (p-value = 0,000; OR = 19,122; 95% Cl = 3, 857-94, 814) and the variable which is the protective factor is suitability with the use of dose (p-value = 0.036; OR = 0, 120; 95% Cl = 0.016-0.874). The conclusion in this study, most of the risk factors for abnormal hemoglobin levels are the variable of pesticide exposure and the most dominant factor in reducing hemoglobin levels is the number of types of pesticides.
