HAZOPS is one of risk analysis which has a creativity interaction, systematic, and cooperate in identification problem. Gas block station (SPG) XI Lembak have a decission as a natural gas and oil producer in Prabumulih field. Risk can occur any time and threaten production, occupational health and safety of worker and enviromental pollution. There are so many equipment who support the process production such as separator, scrubber, compressor ,tank and transfer pump. The purpose of this study was to analyze the deviation, cause, concequences, safeguard, risk rangking and gave a recommendation in every scenario. This study was use qualitative approach. Study informants were five people. The result of this study shown if every node can experience deviations in each parameter like no flow, less flow more flow, reverse flow, high pressure, less presure, high temperature, high level, no level and less level. Causes from deviaton can be originated from valve’s damaged, instrument damaged, equipment’s damaged, safeguard’s damaged, interference with production equipment, production decrease and human eror. Deviations are No flow, less flow, more flow, reverse flow, high pressure, low pressure, high temperatur, high level, dan low level, no level. A scenario of impcat from deviation is, company will loss in equipments, human, and environment such as built up pressure at upstream, explode, fire, oill spill, company’s image and loss sales. Separator, scrubber, compressor, and tank have a safeguard and installed at equipment. Based on research, was found two equipment which have a high risk category, it was show in scrubber and tank
