Hypertension or high blood pressure is one of the highest causes of death in the world. Lifestyle changes result in a change in the trend of hypertension events that generally occur in old age and then shift in adolescens. This is due to risky adolescents behavior such as smoking, lack of exercise and rarely consume fruits and vegetables. Objective the purpose of this study was to assess hypertension prevention behavior in adolescents based on theory of planned behavior. Method: This study uses a cross sectional study approach. The population of this study was 22,439 people whose samples came from teenagers who were registered as students and students of Palembang City High School aged 17-19 years as many as 209 people. Primary data collection was obtained through a questionnaire conducted in January to February 2020 using the cluster random sampling method. Data analysis was performed univariate, bivariate and multivariate. Results: The results showed that the intention of adolescents to prevent hypertension was 52,6% and the strong theory of planned behavior variable was the norm of belief that was equal to 64,6%. Bivariate analysis showed that behavior beliefs (p=0,006); OR=2,281; CI(1,297-4,011), evaluation of behavior beliefs (p=0,422); OR=1,302; CI(0,752-2,254), normative beliefs (p=0,031);OR= 1,957; CI(1,101-3,477), motivation to fulfill (p=0,002); OR=2,613; CI(1,470-4,647), belief control (p=0,001); OR=2,8; CI(1,586-4,942) and perception of strength (p=0,084); OR=1,682; CI(0,973-2,909). Conclusion: Behavioral beliefs, evaluation of behavioral beliefs, norm beliefs and control of beliefs are factors related to the intention to prevent hypertension. The most influential factor with the intention to prevent hypertension is belief control
