Stroke is the second leading cause of death and the third leading cause of disability worldwide. American Heart Association recommends using a stroke severity scale such as the National Institutes of Health Stroke Scale (NIHSS) for evaluation. This study was conducted to identify a description of the degree of stroke severity using NIHSS based on the stroke type, onset, and risk factors of respondents in the stroke unit, dr. Mohammad Hoesin. This study used an observational descriptive design. Sampling was done by total sampling technique with a sample size of 24. Data were taken from medical records. From medical records, it was found that strokes were mostly experienced by the age group 41-65 years (62.5%), men (54.2%), respondents with the latest high school education level/equivalent (62.5%), respondents who did not work (50%).
