Background: A fracture is a break in the continuity of bone tissue. Fractures in children most often occur due to fall. Usually, children who experience a fracture will feel pain. One of medical treatments that can be used to treat fractures is ORIF operative therapy. Fracture patients after ORIF surgery often complain of pain. One of therapies that can help reduce pain is a combination of deep breathing relaxation therapy and cryotherapy with a cold compress method using an ice gel pack. Objective: To describe the results of pediatric nursing practice which focuses on nursing care for children with extremity fractures after ORIF surgery in accordance with the study of evidence-based learning. Method: The method used was a descriptive case study approach and a journal review of the patients with fractures after ORIF surgery. Results: The results of the study showed that all three managed patients had undergone ORIF surgery, with the same complaints, namely pain on different scales. There were 4 nursing problems that emerged in the three patients. The therapy given in this scientific nursing care was a combination of deep breathing relaxation therapy and cryotherapy with cold compress method using an ice gel pack with a duration of 15 minutes for 3 consecutive days. The combination of deep breathing relaxation therapy with cryotherapy used as additional therapy could help reduce the scale of pain of the patients. Discussion: The combination of deep breathing relaxation therapy and cryotherapy could help reduce the pain scale of post-ORIF fracture patients by stimulating the body to produce endorphins to inhibit the transmission of pain impulses to the brain, and it could reduce the sensation of pain resulting in decreased pain scale felt by the patients. Conclusion: Giving the combination of deep breathing relaxation therapy and cryotherapy as additional therapy in dealing with pain problems in pediatric patients with fractures after ORIF surgery could help accelerate the reduction of pain scale of patients. This intervention can be an alternative to help reduce pain in pediatric patients with fractures after ORIF.
