Low back pain (LBP) is a complaint characterized by pain, muscle tension, or stiffness in the area between the 12th rib and the iliac crest, which can reduce quality of life and productivity. Globally, LBP affected approximately 619 million people in 2020, including 104 million cases in Southeast Asia. Islamic school students are a high-risk population due to their intense study activities, long periods of sitting, poor ergonomic sitting positions, and high academic pressure. This study aims to analyze the risk factors that influence the incidence of LBP among Izzuddin Palembang santri. This study is an analytical observational study with a cross-sectional design involving 76 santr who met the inclusion and exclusion criteria. Data were obtained through questionnaires, direct measurements of weight and height, and documentation of sitting positions during study. Stress levels were measured using the Perceived Stress Scale (PSS), while NPB was assessed using the Nordic Musculoskeletal Questionnaire (NMQ). Data analysis was performed using the chi-square or Fisher's exact test, followed by binary logistic regression using SPSS. The results showed that 50.67% of santri experienced NPB, with 46.10% experiencing mild pain, 15.80% experiencing pain, 3.90% experiencing severe pain, and 34.20% experiencing no pain. Bivariate analysis showed that sitting duration (p=0.023) and sitting position (p=0.039) were significantly associated with the occurrence of NPB, while gender (p=0.106), body mass index (p=0.883), stress level (p=0.375), and family history (p=0.428) were not significantly associated. Binary logistic regression analysis showed that sitting duration was the most influential risk factor for NPB (p=0.029; OR=4.00). In conclusion, the risk factors significantly associated with the occurrence of NPB among Izzuddin Palembang santri were sitting duration and sitting position, with sitting duration being the most influential factor.
