Background: Cigarettes are one of the leading causes of death worldwide. The number of deaths caused by exposure to cigarette smoke reaches 8 million individuals per year. This can occur because cigarette smoke contains more than 7,000 chemical compounds that can cause damage to body cells due to high levels of reactive oxygen species (ROS), thereby increasing oxidative stress that triggers lipid peroxidation and produces MDA products used as biomarkers of oxidative stress. Garlic is known as one of the herbal plants that has natural antioxidant properties because it contains high levels of phytochemicals and organosulfur compounds. This study aims to determine the effectiveness of garlic extract administration in reducing serum MDA levels in Wistar rats. Methods: This study was an in vivo experimental study using a posttest-only controlled group design. This study used 30 male white rats (Rattus novergicus) of the Wistar strain, which were divided into 6 groups, namely the control group, cigarette smoke induction group, vitamin C group, EBP 50 group, EBP 100 group, and EBPC 100 group, with 5 Wistar rats in each group. Each group was exposed to 2 cigarettes for 1 hour every day and administered garlic extract using a sonde for a total of 21 days. Blood serum was collected from the ophthalmic vein, and MDA levels were measured using the TBARS method. The research data were analyzed using the Kruskal-Wallis and Mann-Whitney U tests in SPSS Statistics 24. Results: Cigarette smoke induction increased MDA levels in rat blood serum. Administration of garlic extract at doses of 50 mg/kg BW, 100 mg/kg BW, and a combination of garlic extract and vitamin C did not significantly reduce MDA levels. Conclusion: Garlic extract did not have a significant effect on the increase in MDA levels in the blood serum of Wistar rats.
