Introduction: The ineffective expulsion of sputum can lead to the accumulation of secretions in the airways, resulting in ineffective airway clearance and impaired respiratory function. Management of pulmonary tuberculosis patients with airway clearance problems aims to clear the airways and prevent complications caused by inadequate oxygen supply. Properly performed effective coughing can optimize the removal of secretions from the airways efficiently without causing excessive fatigue. This case study aims to implement nursing care for pulmonary tuberculosis patients by providing effective coughing interventions to address ineffective airway clearance, based on evidence-based practice. Method: A qualitative descriptive method with a case study approach was used on three patients with pulmonary tuberculosis. Results and Discussion: Seven nursing problems were identified in patients with pulmonary tuberculosis, with the main problem being ineffective airway clearance. The intervention provided was effective coughing exercises to help patients remove secretions that can inhibit airflow and cause airway obstruction. The intervention was implemented over three days, and the results showed that airway clearance was partially resolved, as indicated by increased effective coughing and improved breathing frequency. Conclusion: Nursing care was provided for three days, and effective coughing intervention had a positive effect on improving airway clearance in three pulmonary tuberculosis patients at RSUP Dr. Mohammad Hoesin Palembang.
