PROFIL KADAR ANTI-MÜLLERIAN HORMONE PADA KASUS ENDOMETRIOSIS DENGAN INFERTILITAS PERIODE 2021-2024 DI RSUP DR. MOHAMMAD HOESIN PALEMBANG
Background. Endometriosis is a chronic inflammatory disease characterized by the presence of endometrial tissue outside the uterine cavity, which may cause chronic pelvic pain and infertility. This condition often leads to a reduction in ovarian reserve due to inflammatory processes, fibrosis, and follicular damage. One of the indicators used to assess ovarian reserve is the Anti-Müllerian Hormone (AMH), produced by granulosa cells of antral and preantral follicles. A decrease in AMH levels may reflect a reduction in follicle count and the ovary’s reproductive capacity. This study aims to determine the distribution of AMH levels based on age, age at menarche, body mass index (BMI), menstrual duration, and infertility duration among patients with endometriosis-associated infertility at Dr. Mohammad Hoesin General Hospital Palembang. Methods. This was a descriptive observational study with a case series design. Data were analyzed using univariate analysis and presented in the form of frequency distributions to describe the clinical characteristics of the subjects Result. A total of 82 patients met the inclusion criteria. Most patients were aged 25–34 years (50%) with AMH levels categorized as normo-responder (25.6%). Based on age at menarche, the majority were within the range of 8–15 years (98.8%), with AMH levels mostly in the poor responder category (47.6%). According to BMI, most patients had normal BMI (48.8%) with AMH levels in the poor responder category (24.4%). The majority of patients had menstrual duration ≤8 days (98.8%) and infertility duration >8 years (36.6%), both predominantly in the poor responder category. Overall, lower AMH levels were observed among patients of older age and with longer infertility duration. Conclusion. AMH levels in patients with endometriosis and infertility varied across all clinical characteristics, with a tendency toward lower levels in older patients and those with prolonged infertility. These findings suggest that advancing age and longer infertility duration may reflect a decrease in ovarian reserve among women with endometriosis. Keywords.
