Background: Traumatic brain injury (TBI) is one of the leading causes of morbidity and mortality among critically ill patients. Coagulation abnormalities are frequently observed in TBI patients and may significantly influence clinical outcomes, including intensive care unit (ICU) length of stay, duration of mechanical ventilation, and mortality. This study aims to evaluate the association between coagulation parameters with ICU length of stay, duration of mechanical ventilation, and mortality in TBI patients. Methods: This was an analytical observational study with a retrospective cohort design involving TBI patients admitted to the intensive care unit of Mohammad Hoesin General Hospital, Palembang. Data on patient characteristics (age, sex, and GCS Score), coagulation profiles, and clinical outcomes were analyzed. Associations between variables were tested using Chi-square and Fisher’s exact test for bivariate analysis, and logistic regression for multivariate analysis with a p-value < 0.05 was considered statistically significant. Results: Most TBI patients were under 60 years old (83.33%), predominantly male (81.25%), and had a GCS admission score of 9–12 (moderate TBI) in 43.75% of cases. Patients with abnormal PT had a 2.41-fold higher risk of ICU stay ≥7 days compared to those with normal PT (p = 0.017, 95% CI: 1.11–5.14). Patients with abnormal PT had a 3.53-fold higher risk of requiring mechanical ventilation ≥7 days compared to patients with normal PT (p = 0.012, 95% CI: 1.2–10.31). Patients with abnormal INR had a 4-fold higher risk of mortality compared to those with normal INR (p = 0.003, 95% CI: 1.49–10.7). Conclusion: Coagulation disorders, particularly abnormal PT and INR, are significant factors affecting clinical outcomes in TBI patients. Close monitoring and early management of coagulation abnormalities are essential to improve clinical outcomes and reduce mortality risk in TBI.
