Latar belakang: Salah satu komplikasi utama Chronic Kidney Disease yaitu hipervolemia akibat penurunan fungsi ginjal dan rendahnya kadar albumin yang menyebabkan edema pada anggota tubuh. Kondisi ini dapat menimbulkan ketidaknyamanan, penurunan mobilitas, serta mengganggu aktivitas sehari-hari. Salah satu tatalaksana nonfarmakologis untuk mengurangi edema yaitu terapi kombinasi foot massage dan foot elevation 30º yang membantu memperlancar sirkulasi dan mengurangi penumpukan cairan. Tujuan: Menggambarkan pelaksanaan asuhan keperawatan pada pasien CKD dan mengaplikasikan kombinasi foot massage dan foot elevation 30º untuk menurunkan derajat edema pada masalah keperawatan hipervolemia. Metode: Penelitian ini menggunakan metode deskriptif dalam bentuk studi kasus pada 3 pasien CKD dengan masalah keperawatan hipervolemia yang dilakukan foot massage selama 20 menit dan foot elevation 30º selama 3-5 menit sebanyak 2 sesi selama 3 hari. Hasil: Hasil intervensi kombinasi foot massage dan foot elevation 30º selama 3 hari menunjukkan adanya penurunan derajat edema pada ekstremitas bawah dari edema derajat 2 menjadi edema derajat 1. Pembahasan: Foot massage akan meningkatkan aliran vena dan limpatik dari kaki serta mengurangi tekanan hidrostatik intravena, yang mengakibatkan cairan plasma ke ruang interstitium dan cairan yang beredar akan kembali ke vena sehingga edema dapat berkurang. Elevasi kaki merupakan gerakan meninggikan kaki dengan memanfaatkan gravitasi untuk meningkatkan aliran balik vena dan mengurangi tekanan pada pembuluh darah. Kesimpulan: Penerapan terapi kombinasi foot massage dan foot elevation 30º efektif sebagai terapi pendukung dalam menurunkan derajat edema pada pasien CKD. Terapi ini tergolong aman, mudah dilakukan dan minim efek samping sehingga dapat dijadikan sebagai intervensi mandiri keperawatan untuk menurunkan derajat edema. Kata Kunci : CKD, Edema, Foot Massage, Foot Elevation 30º, HipervolemiaBackground: One of the main complications of Chronic Kidney Disease is hypervolemia due to decreased kidney function and low albumin levels, which causes edema in the limbs. This condition can cause discomfort, decreased mobility, and interfere with daily activities. One of the non-pharmacological treatments to reduce edema is a combination of foot massage and 30º foot elevation therapy which helps improve circulation and reduce fluid buildup. Objective: To describe the implementation of nursing care in CKD patients and apply a combination of foot massage and 30º foot elevation to reduce the degree of edema in hypervolemia nursing problems. Method: This study used a descriptive method in the form of a case study on 3 CKD patients with hypervolemia nursing problems who underwent foot massage for 20 minutes and 30º foot elevation for 3-5 minutes for 2 sessions over 3 days. Results: The results of the intervention of a combination of foot massage and 30º foot elevation for 3 days showed that a decrease in the degree of edema in the lower extremities from grade 2 edema to grade 1 edema. Discussion: Foot massage will increases venous and lymphatic flow from the legs and reduce intravenous hydrostatic pressure, which results in plasma fluid to the interstitial space and circulating fluid will return to the veins so that edema can be reduced. Foot elevation is a movement that elevates the feet using gravity to increase venous return and reduce pressure on the blood vessels. Conclusion: Implementation of the combination of foot massage and 30º foot elevation therapy is effective as a supportive therapy in reducing edema degree in CKD patients. This therapy is considered safe, easy to perform, and has minimal side effects, making it a suitable as an independent nursing intervention to reduce edema degree.
