Background: The most common problem found in pleural effusion patients is ineffective breathing patterns due to fluid buildup in the pleural cavity which decreases lung expansion and inhibits ventilation. This condition is characterized by shortness of breath, increased breathing frequency, and the use of the respiratory support muscles. One of the interventions that can be applied to address this problem is a combination of pursed lip breathing and the Fowler position. Objective: To apply nursing care to patients with pleural effusion with pursed lip breathing and fowler position interventions on ineffective breathing pattern nursing problems. Methods: The research method was descriptive with a case study approach of three pleural effusion patients. Results: An assessment of the three patients showed the complaints in the form of shortness of breath, increased breathing frequency, elongated expiratory phase, use of the respiratory support muscles, complaining of fatigue, increased tightness of activity, lack of appetite, and pain. There were 6 nursing problems established in pleural effusion patients showing that the main nursing problem was ineffective breathing patterns. Discussion: The administration of pursed lip breathing techniques and fowler positions is an additional intervention given to improve breathing patterns. The administration of this technique was given for 15 minutes for three consecutive days. The pursed lip breathing helped slow expiration, while the fowler position facilitated lung expansion and improved breathing comfort. Providing both of these interventions helped improve ventilation by lowering dyspnea, breathing frequency, and the use of the respiratory support muscles, as well as increasing the oxygen saturation to improve the breathing patterns. Conclusion: The combination of pursed lip breathing technique and Fowler's position is effective in overcoming the nursing problem of ineffective breathing patterns in patients with pleural effusion
