Latar Belakang : Penyakit ST-Elevation Myocardial Infarction (STEMI) merupakan salah satu penyebab utama kematian global, yang membutuhkan penanganan cepat melalui Intervensi Koroner Perkutan (IKP) primer. Keterlambatan dalam tindakan reperfusi masih menjadi tantangan di Indonesia, termasuk di RSUP Dr. Mohammad Hoesin Palembang. Penelitian ini bertujuan mengevaluasi waktu pencapaian Intervensi Koroner Perkutan Primer (IKP primer) pada pasien STEMI serta mengidentifikasi faktor-faktor keterlambatan dari sisi pasien dan sistem. Metode : Penelitian ini menggunakan metode deskriptif retrospektif dengan data pasien STEMI yang dirawat selama Januari-Desember 2023. Variabel utama yang dievaluasi meliputi door-to-wire crossing time, door-to-needle time, total ischemic time, serta faktor pasien dan sistem yang memengaruhi keterlambatan. Analisis dilakukan secara deskriptif untuk menentukan distribusi waktu dan pola keterlambatan. Hasil : Hasil menunjukkan keterlambatan banyak terjadi baik pada pasien yang datang langsung ke rumah sakit dengan fasilitas Intervensi Koroner Perkutan Primer (IKP primer)maupun pasien transfer dari rumah sakit non-PCI. Faktor pasien meliputi kurangnya kesadaran terhadap gejala STEMI dan lambatnya mencari pertolongan medis, sementara faktor sistem mencakup keterbatasan fasilitas, alur rujukan yang lambat, dan jumlah tenaga medis yang kurang memadai. Kesimpulan :Waktu pencapaian Intervensi Koroner Perkutan Primer (IKP primer)pada pasien STEMI di RSUP Dr. Mohammad Hoesin belum optimal. Upaya perbaikan meliputi edukasi masyarakat, optimalisasi sistem rujukan, dan peningkatan kapasitas rumah sakit untuk mempercepat penanganan. Penelitian ini menjadi acuan untuk meningkatkan kualitas pelayanan STEMI di Indonesia. ABSTRACT EVALUATION OF TIME TO ACHIEVE PRIMARY PERCUTANEOUS CORONARY INTERVENTION IN STEMI PATIENTS AT RSMH PALEMBANG (Fahira Mohammad, November 28, 2024, 108 pages) Faculty of Medicine, Universitas Sriwijaya, Palembang Email: fahiramhmmd@gmail.com Background: ST-Elevation Myocardial Infarction (STEMI) is a leading cause of global mortality, requiring prompt management through Primary Percutaneous Coronary Intervention (PPCI). Delays in reperfusion therapy remain a notable challenge in Indonesia, including at RSUP Dr. Mohammad Hoesin Palembang. This study aims to evaluate the time required to achieve PPCI in STEMI patients and identify delay factors related to patients and healthcare systems. Methods: This study used a descriptive retrospective design with data from STEMI patients treated from January to December 2023. The main variables evaluated were door-to-wire crossing time, door-to-needle time, total ischemic time, and patient- and system-related delay factors. Descriptive analysis was performed to determine the distribution and patterns of delays. Results: The findings revealed considerable delays both in patients who presented directly to hospitals with PPCI facilities and in transferred patients from non-PCI hospitals. Patient-related factors included a lack of awareness of STEMI symptoms and delayed seeking of medical assistance. System-related factors involved limited hospital facilities, slow referral pathways, and insufficient numbers of trained healthcare professionals. Conclusion: The time required to achieve PPCI in STEMI patients at RSUP Dr. Mohammad Hoesin remains suboptimal. Improvements are needed through public education, optimization of referral systems, and enhancement of hospital capacity to expedite treatment. This study serves as a reference for improving STEMI care quality in Indonesia.
