HUBUNGAN USIA, JENIS KELAMIN, DAN RIWAYAT DIABETES MELLITUS DENGAN KEJADIAN FISTULA PERIANAL DI RSUP DR. MOHAMMAD HOESIN PALEMBANG PERIODE 2020-2023
Perianal fistula is a pathological channel connecting the anus to the perianal skin area. A clear dominance is observed among young adults (mean age: 40 years) dan males, with diabetes mellitus identified as its predisposing factor. This study aims to describe the characteristics of perianal fistula patients and analyze the relationship between age, gender, and history of diabetes mellitus with perianal fistula at Dr. Mohammad Hoesin Hospital Palembang. An observational study with cross sectional approach was conducted on 48 patients with perianal fistula. The proportion of perianal fistulas was found to be 27.27%. Most patients were aged 30–50 years (52.08%), male (81.25%), complains of openings and discharge (27.08%), with a single external opening (79.17%) located posteriorly (79.17%), and had perianal abscess (43.75%). Fistulectomy was the most frequently performed procedure (66.67%) and the majority of patients reported no prior history of perianal fistula surgery (58.33%). Bivariate analysis revealed a significant relationship between age (p=0.027) and gender (p=0.012), whereas diabetes mellitus was not found to be a significant factor (p=259). Multivariate analysis identified age as the most strongly associated factor with the perianal fistula incidence (p=0.027). Age and gender were found to have a significant impact on the occurrence of perianal fistulas. This relationship is explained by the cryptoglandular theory. The potential impact of diabetes mellitus, which approached statistical significance, suggests that it may be clinically relevant with perianal fistula incidence. As such, further studies are needed to analyze its role in the development of perianal fistulas.
