PROFIL FUNGSI JANTUNG PADA ANAK LEUKEMIA LIMFOBLASTIK AKUT DENGAN KEMOTERAPI ANTRASIKLIN DI RS MOHAMMAD HOESIN PALEMBANG
Acute lymphoblastic leukemia (ALL) is a hematological malignancy that frequently occurs in children, with a peak incidence between 2–5 years of age. Treatment for ALL often involves chemotherapy using anticancer drugs to destroy cancer cells. One commonly used drug class is anthracyclines, which are known for their cardiotoxic effects. Anthracycline-induced cardiotoxicity can damage the heart muscle, leading to impaired cardiac function. This study aimed to determine the cardiac function profile of children with ALL undergoing anthracycline chemotherapy at Mohammad Hoesin Hospital, Palembang. This was a descriptive observational study with a cross-sectional design. Subjects were selected using a consecutive sampling technique. Primary data were obtained through interviews and echocardiographic examinations. Data analysis was conducted using SPSS version 27.0 and presented in the form of frequency distributions. The study included 31 subjects. Most patients were in the age group of 1– 10 years (64.5%), were male (71%), had good nutritional status (71%), were classified as high-risk ALL (74.2%), were in the maintenance phase (51.6%), had treatment durations > 7 weeks (64.5%), used the anthracycline daunorubicin (100%), and had cumulative doses ≥ 120 mg/m2 (58,1%). Echocardiographic profiles showed LVEF > 55% (100%), LVFS ≥ 28% (100%), MV E/A ratio of 1.5– 2.3 (64.5%), TAPSE ≥ 15 mm (100%). Findings of heart abnormalities include minimal pericardial effusion (19.3%) and tricuspid regurgitation (3.2%). All study subjects exhibited normal biventricular systolic function, although some patients demonstrated altered left ventricular diastolic function, including both reductions and increases.
