Headache was one of the most common symptoms experienced by hypertensive patients, especially when blood pressure rose. Pain management could be done through non-pharmacological therapy to support medical treatment, one of which was complementary therapy. This study aimed to determine the differences in pain scales and effectiveness between Slow Stroke Back Massage (SSBM) therapy and warm compresses on the neck for headaches in hypertensive patients in the working area of the Social Health Center. The design used was a quasi-experimental study with a pretest-posttest approach without a control group. A total of 30 respondents were selected using purposive sampling and were divided into two intervention groups (15 people each). The intervention was administered for 10–15 minutes, once daily for three days. Pain levels were measured using the Numeric Rating Scale (NRS). The wilcoxon test shows a significant difference in pain levels before and after the intervention within each group (p = 0.001). The mann-whitney test shows a significant difference between the two groups after the intervention (p = 0.011). The n-gain value for the warm compress group is classified as moderately effective (71.93%), while the SSBM group is less effective (40.28%). The difference in effectiveness between the two therapies is influenced by their respective physiological mechanisms. Warm compresses on the neck increase local tissue temperature, triggering vasodilation and muscle relaxation, thereby reducing pain more quickly. Conversely, slow stroke back massage therapy produces an analgesic effect gradually through stimulation of sensory receptors and the release of endorphins, so the pain-reducing response takes longer to occur.
