Background: Dry eye syndrome is a common ocular complication in patients with type 2 diabetes mellitus, potentially impairing vision and quality of life. Sodium hyaluronate (SH) and carboxymethylcellulose (CMC) are commonly used topical treatments, but comparative data on their effectiveness and the influence of glycemic control are limited. This study objectives is to evaluate and compare the effectiveness of SH and CMC in improving clinical symptoms and signs of dry eye syndrome in type 2 diabetes patients, and to assess the correlation between HbA1c levels and therapeutic response. Methods: This randomized controlled trial (RCT) study involved patients type 2 diabetes with dry eye syndrome. Subjects were divided into two treatment groups: SH and CMC. Evaluations were performed at baseline, week 4, week 6 and week 8 using the Ocular Surface Disease Index (OSDI), Tear Film Break-Up Time (TBUT), and Schirmer’s test. Correlation analysis was conducted to examine the relationship between HbA1c and clinical improvement. Results: Both groups showed significant improvements in OSDI, TBUT, and Schirmer scores by week 4 (p
