Background: Primary Open-Angle Glaucoma (POAG) and high myopia are two ophthalmological conditions that can lead to thinning of the retinal nerve fiber layer (RNFL). POAG is characterized by increased intraocular pressure (IOP), contributing to optic neuropathy and visual field defects. Meanwhile, high myopia is associated with elongation of the eyeball, leading to structural changes in the RNFL. This study aims to compare RNFL characteristics in patients with POAG and high myopia to understand the thinning patterns in each condition. Objective: To determine the characteristics of the retinal nerve fiber layer (RNFL) in patients with primary open-angle glaucoma and high myopia. Methods: This observational study utilized a cross-sectional design conducted at the Glaucoma and Refraction Subdivision of the Ophthalmology Clinic at Dr. Mohammad Hoesin Central General Hospital Palembang and the South Sumatra Provincial Eye Hospital. The study subjects were divided into two groups: POAG patients and high myopia patients, with a total of 50 eyes in each group. RNFL thickness was measured using optical coherence tomography (OCT) and analyzed across four quadrants (superior, inferior, nasal, and temporal). Results: The study results demonstrated a significant difference in RNFL thickness between POAG and high myopia patients (p = 0.01). The average RNFL thickness in POAG patients was lower than in high myopia patients. In POAG, RNFL thinning was more pronounced in the superior and inferior quadrants, consistent with the characteristic pattern of glaucomatous optic neuropathy. Conversely, high myopia patients exhibited more diffuse RNFL thinning, primarily affecting the temporal and nasal quadrants due to mechanical stretching caused by eyeball elongation. Conclusion: RNFL thinning in POAG follows a specific pattern, predominantly affecting the superior and inferior quadrants. In contrast, high myopia presents with more generalized RNFL thinning, with the temporal and nasal quadrants being the most affected areas.
