Background: Enamel is the outermost and hardest tissue covering the crown of the tooth. Tooth enamel consists of 95% inorganic components in the form of hydroxyapatite crystals, such as calcium and phosphate, and the rest are organic components. If these components are lost, it is called demineralization. The demineralization process causes porosity, resulting in roughness on the tooth surface. Tooth surface roughness can be repaired by the remineralization process. The remineralization process occurs when the acidic pH of the oral cavity is neutralized by adding herbal ingredients to gambir toothpaste as an alternative. Objective: This study aims to determine the effect of gambir toothpaste on tooth surface roughness. Methods: Ten premolar tooth crowns were divided into 2 groups: Group A (Gambir toothpaste), Group B (control). The initial tooth surface roughness value was measured using a Surface Roughness Tester after the demineralization process with 37% phosphoric acid etching. The samples were brushed for 2 minutes twice a day for 14 days, then soaked in artificial saliva and stored in an incubator at 37ºC. Then, it was measured again using a Surface Roughness Tester. Statistical tests used the Paired t-test and Independent t-test. Results: Paired t-test results showed a significant increase in tooth enamel roughness in each group. Independent t-test results showed no significant difference in tooth surface roughness after demineralization and after brushing between groups. Conclusion: Gambir toothpaste has an effect in reducing tooth surface roughness, but there is no significant difference when compared with placebo toothpaste. Key words: Tooth surface roughness, gambir toothpaste, placebo toothpaste.
