Background: Children with acute lymphoblastic leukemia often show psychological reactions such as anxiety, which can lead to resistance to the treatment they are undergoing. Nursing interventions that can be implemented to reduce anxiety levels in pediatric patients with acute lymphoblastic leukemia include puzzle play therapy. Objective: To describe the implementation of nursing care in preschool age children with acute lymphoblastic leukemia and to apply puzzle play therapy to anxiety. Method: A case study on 3 preschool-aged children with acute lymphoblastic leukemia in the Selincah Room, 2nd Floor RSMH Palembang. Results: Assessment of the three child patients revealed complaints such as fever, nausea, vomiting, decreased appetite, weakness, lack of energy, feelings of anxiety, fear, confusion, appearing tense, restlessness, frequent crying, confusion, difficulty sleeping, decreased hemoglobin levels, decreased leukocyte levels, decreased platelet levels, decreased hematocrit levels, decreased neutrophil levels, increased lymphocyte levels, and in the assessment of anxiety levels using the Face Image Scale (FIS), a moderate-severe level of anxiety was found. Nine nursing diagnoses were established with problems including hyperthermia, ineffective peripheral perfusion, nausea, fatigue, sleep pattern disturbance, anxiety, risk of nutritional deficiency, risk of bleeding, and risk of infection. Discussion: The interventions provided include hyperthermia management, circulation care, nausea and vomiting management, energy management, sleep support, nutrition management, bleeding prevention, infection prevention, and anxiety reduction through puzzle play therapy. Puzzle play therapy was conducted for 3 consecutive days for 15-20 minutes, effectively lowering anxiety levels in children. Conclusion: After 3 days of nursing care, all nursing problems were addressed.
