HUBUNGAN KADAR HEMOGLOBIN PRATRANSFUSI DAN KEPATUHAN KONSUMSI TERAPI KELASI BESI DENGAN KUALITAS HIDUP ANAK USIA SEKOLAH PENYANDANG TALASEMIA BETA MAYOR
Beta thalassemia major is a genetic disorder characterized by severe anemia and the need for regular blood transfusions. School-age children with this condition are susceptible to impaired physical, cognitive, emotional, and social development. Unstable pre-transfusion hemoglobin levels and non-compliance with iron chelation therapy can lead to physical fatigue, emotional and social disturbances, and school absence, impacting their quality of life. This study aims to determine the relationship between pre-transfusion hemoglobin levels and adherence to iron chelation therapy with the quality of life of school-age children with beta thalassemia major. This observational, analytical study with a cross-sectional design was conducted in April 2025 in the One Day Care Ward of Dr. Mohammad Hoesin General Hospital, Palembang. The sample consisted of 59 parents/guardians of children selected using purposive sampling. Data collection used the PedsQL questionnaire for quality of life, the MMAS-8 for iron chelation therapy adherence, and medical record data for pre- transfusion hemoglobin levels. The analysis showed a significant association between pre- transfusion hemoglobin levels (p=0.043) and iron chelation therapy adherence (p=0.033) and children's quality of life. Optimal hemoglobin levels can reduce fatigue and improve daily activities in children with beta thalassemia major. Furthermore, adherence to iron chelation therapy plays a crucial role in preventing long-term complications, thereby improving physical and psychosocial quality of life. Therefore, regular monitoring of hemoglobin levels and education on therapy adherence are crucial in improving the quality of life of children with beta thalassemia major.
