The rapid development of information technology has transformed information systems into the backbone of various activities, including health services. The Irma Suryani Midwife Maternity Clinic, which has served more than a hundred patients since 2014, still faces obstacles in the manual registration and counseling process, causing long queues and patient dissatisfaction. This study aims to build a website-based application to facilitate health services at the clinic, improve efficiency, effectiveness, and patient satisfaction. With an integrated information system, patient data can be processed accurately and easily accessed, supporting better decision making. The implementation of this technology is in line with the trend of the Industrial Revolution 4.0 which places technology as the key to improving service quality. It is hoped that this application can improve service quality and support public health in a promotive and preventive manner.
