Low coverage of basic immunization remains a national issue in Indonesia, including the working area of Indralaya Health Center, which has not yet achieved the national standard target. Immunization implementation in the community remains suboptimal, resulting in infants who have not received complete immunization according to the recommended schedule. This study aims to determine the factors associated with the completeness of basic immunization among infants aged 12-24 months. The study used a quantitative design with a cross-sectional approach on 100 mothers with children aged 12-24 months in the working area of public health center Indralaya. The sampling technique was purposive sampling. Data were collected through questionnaires and analyzed with univariate, chi-square, and logistic regression tests The results of the chi-square test showed that education level (p = 0.005) knowledge (p) = 0.000), attitude (p = 0.000) trust (p = 0.000) and family support (p = 0.003) had a significant relationship with the completeness of basic immunization. In contrast, employment status (p = 0.647) and service affordability (p = 0.585) showed insignificant relationships. Based on logistic regression analysis maternal knowledge was the dominant factor with the greatest influence on the completeness of basic immunization with OR 8.344 (95% CI: 2.574-27.050). followed by family support with OR 4.637 (95% CI 1.172-18.348), maternal trust with OR 4423 (95% CI: 1.186-16 494), and maternal attitude with OR 3.445 (95% CI 1.110-10.696) Efforts to increase basic immunization should be supported by continuous education, family involvement, provision of accurate information, and interactive counseling to manage these factors and increase basic immunization coverage.
