Acute Respiratory Infection (ARI) is one of the diseases caused by indoor air pollution. The prevalence of ARI in toddlers aged 0-59 months in the Talawi Health Center working area in West Sumatra was 14.52% in 2022 and 713 in 2023. This study aims to determine the relationship between environmental factors (the use of mosquito coils, the presence of smokers in the house, and the physical condition of the house) with the incidence of ARI in toddlers in the working area of Puskesmas Talawi West Sumatra. This study used a cross sectional research design with cluster random sampling techniques on 130 respondents who had toddlers with an age range of 0-59 months. Data analysis in this study used chi-square test and multiple logistic regression test. Based on the results of the analysis obtained 44.6% of toddlers aged 0-59 months in the Talawi Health Center working area of West Sumatra experienced ARI. The results of the chi-square test analysis showed that there was a relationship between the presence of smokers in the house (P=0.000), occupancy density (P=0.000), and the type of wall of the house (P=0.000) with the incidence of ARI in toddlers, and there was no relationship between the use of mosquito coils, lighting, ventilation, and the type of floor of the house with the incidence of ARI in toddlers. The results of multiple logistic regression test analysis showed that the variable of the presence of smokers in the house was the most influential factor on the incidence of ARI (PR 95% CI = 10.386) after being controlled by the variables of mosquito coil use, occupancy density, ventilation, and type of house wall. People who smoke should clean themselves and change their clothes before approaching or holding toddlers.
