Schizophrenia is a chronic mental disorder with a high relapse rate, which impacts patients, families and the health system. The relapse of schizophrenia patients at Ernaldi Bahar Hospital in South Sumatra Province remains a challenge, despite the implementation of various policies to reduce the rate of re-hospitalization. This study aims to analyze risk factors for relapse in schizophrenia patients and evaluate policies that have been implemented in an effort to reduce relapse using a case-control design involving schizophrenia patients at Ernaldi Bahar Hospital in the period January-June 2024. The study sample was 54 cases and 54 controls. Sample withdrawal in the control group using the “non matching” technique. Data were collected through questionnaires and medical record analysis, and analyzed using multiple logistic regression. The results of bivariate analysis showed that there was a significant relationship between the level of education (OR = 13.3), access and distance to health care facilities (OR = 7.5), family support (OR = 7.6), health worker support (OR = 5.1), and adherence to taking medication (OR = 4.1) with the incidence of relapse of schizophrenia patients. While the variables of social support and marital status are confounding variables. The variables of age, gender and occupation did not have a significant relationship with the incidence of relapse. The dominant variable associated with the incidence of relapse in schizophrenia patients is the education variable. Relapse of schizophrenia patients is influenced by adherence to taking medication, access and distance to health services, family support, health worker support, and education level.
