Alcohol abuse is a global health problem that damages the brain, particularly the prefrontal cortex area, through increased inflammation and apoptosis mediated by Caspase-3. Lifestyle interventions such as calorie restriction (CR) and exercise have the potential to provide neuroprotective effects. This study aims to evaluate the effect of CR and exercise on Caspase-3 expression in the brains of alcohol-consuming rats. This experimental study used a post-test only controlled group design with 30 Wistar rats divided into five groups: control group, alcohol group, alcohol and CR group, alcohol and exercise group, and alcohol, CR and exercise group. Alcohol treatment was given as 20% v/v ethanol in drinking water, CR was a 20% reduction of the standard diet, and exercise was involuntary motorised hurdle running wheel aerobic exercise at a speed of 10-12 m/min for 30 minutes per session. Experiments were carried out for six weeks when brain samples were harvested and analysed immunohistochemically. Data were measured using ImageJ and analysed statistically. The results showed that alcohol significantly increased Caspase-3 expression (mean difference 15.586; p = 0.000). Both CR and exercise individually reduced alcohol-induced Caspase-3 expression (p = 0.000, difference -16.428 AU), while the combination of both interventions resulted in an even greater reduction (p = 0.000, difference -24.954 AU). However, the combination of CR and exercise did not eliminate the effects of alcohol. These findings indicate that although these lifestyle interventions provide a neuroprotective effect, alcohol consumption still has a significant negative impact on brain health.
