Diarrhea is the leading cause of under-five deaths in the world, accounting for 18% of total deaths with more than 5,000 cases per day. In Indonesia, the number of under-five deaths due to diarrhea in South Sumatra reached 29,437, with Palembang City as the highest contributor (10,773 cases). The prevalence of diarrhea in toddlers in the Social Health Center working area in 2024 was 60.6%. This study aims to analyze the relationship between hand washing with soap (HWWS) behavior, environmental sanitation, and food sanitation with the incidence of diarrhea in toddlers in settlements around the Sukawinatan landfill. The research method used a cross-sectional design with simple random sampling, involving 102 respondents. Data analysis was conducted univariately, bivariately (chi-square test), and multivariately (multiple logistic regression test). The results showed that food storage, wastewater disposal facilities, and garbage disposal facilities did not have a significant relationship with the incidence of diarrhea (p-value > 0.05). Meanwhile, HWWS behavior, food preparation method, clean water availability, and latrine facilities had a significant association with the incidence of diarrhea. Multivariate analysis found that HWWS behavior was the most dominant factor in influencing the incidence of diarrhea among under-fives (p-value = 0.002; PR = 5.729; 95%CI = 1.902-17.257). In conclusion, HWWS behavior strongly influences the incidence of diarrhea in toddlers. Therefore, the community is advised to improve clean and healthy living habits, such as keeping nails clean and providing soap for hand washing to prevent the spread of disease and reduce the risk of diarrhea.
