The Referral Back Program (PRB) is an effort by the National Health Insurance Fund to sustainably manage chronic diseases through the services of the Community Health Center. This program is designed so that patients who were previously referred to advanced health facilities can return to the Community Health Center for care, thus helping the Community Health Center to improve its gatekeeper function and the efficiency of health care costs in the FKRTL. This study aims to analyze the implementation of PRB at Community Health Centers in Palembang City using a qualitative case study approach. Data were collected through in-depth interviews and focus group discussions, and informants were identified using the maximum variation method, namely 3 expert informants (BPJS Kesehatan), 12 key informants (PIC officers and Community Health Center doctors), and 6 supporting informants (PRB patients). The data were analyzed using thematic analysis techniques. The results of the study showed that the implementation of PRB in Palembang City Community Health Center was constrained by double burden and limited human resources, lack of training for community health center officers, limited budget, and no PRB corner. The distribution of Referral Back Letters and Control Books was not optimal, hampering record keeping and coordination. The lack of medicines in network pharmacies and the lack of digitization of referral letters and PRB logbooks are challenges. In addition, there is a lack of feedback and reporting from health centers to BPJS Kesehatan, so the performance of active PRB participants is still low. There is a need to support health worker training and more intensive socialization to strengthen the role of health centers as supervisors of participant treatment through health cadres and network pharmacies. It is expected that the program will operate more optimally and reach its target number of active participants.
