Background: Patients at high risk of caries or those who have developed early carious lesions require appropriate and immediate treatment. Untreated carious lesions can lead to extensive tooth structure damage and require invasive procedures. Prevention and management of carious lesions on the occlusal surface can be achieved by applying pit and fissure sealants, one of which is a resin-based fissure sealant. Objective: To determine the effectiveness of resin-based pit and fissure sealant in preventing the formation and halting the progression of carious lesions through a literature review. Methods: This study is a literature review with qualitative analysis. Journals obtained through Google Scholar, PubMed, and ScienceDirect databases were selected, and then bias analysis and data extraction were conducted. Results: This study utilized 9 randomized controlled trial journals and 4 cohort study journals. Seven journals concluded that resin-based pit and fissure sealants effectively prevent the formation and halt the progression of caries, as well as are superior to glass ionomer sealants. One journal identified differences in effectiveness among types of resin-based pit and fissure sealants. And five journals found comparable effectiveness between resin-based sealants and glass ionomer sealants or fluoride varnish. Conclusion: Resin-based pit and fissure sealants are effective in preventing caries in individuals at high risk and halting the progression of individuals with early non-cavitated occlusal caries. The retention of the sealant on the tooth surface influences the effectiveness of resin-based pit and fissure sealants in preventing the formation and halting the progression of caries.
