According to the results of the 2023 Indonesian Health Survey, the prevalence of ARI in children under five has almost tripled compared to the 2018 Basic Health Research data, from 12.8% to 34.2%. The role of health facilities and health workers in improving promotive and preventive services can reduce ARI cases in children under five. This study aims to determine the distribution of ARI incidence among children under five in West Lampung District with spatial analysis to describe the pattern of case distribution. This research is descriptive with an ecological study design. The population group included 15 sub-districts in West Lampung District. Secondary data were obtained from the Health Office and the Central Bureau of Statistics of West Lampung District. Data were processed descriptively using spatial analysis with the QGIS (Quantum GIS) application through overlay and buffering methods. The results showed that ARI cases among children under five were spread across 8 sub-districts with Lumbok Seminung sub-district recording the highest cases (22 cases) with male children under five being the most dominantly affected with a ratio of 1.45:1. The highest population density was in Kebun Tebu sub-district (347.52 people/km2) with a high category. Most areas have been reached by health facilities such as community health center (5 km) and hospitals (6 km), although there are still sub-districts that have not been reached. The number of doctors is highest in Kebun Tebu sub-district (4 doctors), the number of midwives is highest in Way Tenong sub-district (61 midwives), and the number of integrated service post is highest in Balik Bukit sub-district (27 integrated service post). It is expected that the role of community health center and medical personnel will be increased in preventive efforts and strengthening the ISPA Routine Data Reporting System to reduce the incidence and morbidity of ISPA among children under five.
