KAJIAN DRUG RELATED PROBLEMS(DRPs) PADA PASIEN CONGESTIVE HEART FAILURE (CHF) DI RSI SITI KHADIJAH PALEMBANG
Congestive Heart Failure (CHF) is a progressive clinical condition that causes the inability of the heart to pump blood optimally, resulting in impaired perfusion of vital organs. The main therapeutic regimens used in CHF patients include diuretics, ACE inhibitors/ARNI beta-blockers, antiplatelets. CHF patients often undergo polypharmacy therapy which increases the risk of Drug Related Problems (DRPs) that can potentially hinder the achievement of optimal therapeutic outcomes. The purpose of this study was to identify the types of DRPs that occurred in hospitalized CHF patients at RSI Siti Khadijah Palembang. This study used a prospective descriptive design with cross sectional. Data were collected through direct observation and review of medical records of CHF patients from December 2024 to January 2025. DRPs were classified using Pharmaceutical Care Network Europe (PCNE). Data analysis was done quantitatively and presented as a percentage. The results showed that Drug Related Problems (DRPs) that occurred in Congestive Heart Failure patients were drug interactions (20%) and polypharmacy (63.9%).
