Diphtheria is an infection caused by bacteria of the Corynebacterium species, particularly Corynebacterium diphtheriae (C. diphtheriae). Since 2018, the WHO has reported an increase in cases in several places, including Indonesia, India, South America, and Africa. Given the high incidence of diphtheria and the severity of its complications, as well as the numerous risk factors that may be associated with diphtheria, a study on the risk factors associated with diphtheria in diphtheria-endemic countries is needed. The aim of this study is to determine the risk factors for diphtheria in endemic countries through a systematic review. The study was conducted from July to October 2024 with a sample of 51 selected literature from Google Scholar, Pubmed, Science Direct, and ProQuest. Host factors (immunization status, age, gender, contact, economic status, nutritional status, and knowledge) and environmental factors (immunization coverage, place of residence, housing conditions, and conflict areas) were risk factors associated with diphtheria in diphtheria-endemic countries. Among these factors, immunization status was the factor with the largest proportion in supporting the occurrence of diphtheria in endemic countries, namely 42 studies (82.3%) out of 51 studies that discussed the relationship between immunization status and diphtheria and were statistically proven. The government can take a multi-sectoral approach to increase immunization coverage and pay more attention to environmental factors
