The government has set four targets as the main focus of PIS-PK (Healthy Indonesia Program with Family Approach), which include reducing maternal and neonatal mortality rates, reducing the prevalence of stunting due to malnutrition, and health interventions to control communicable and non-communicable diseases. This study aimed to describe the family health status based on the Healthy Family Index (IKS) and analyze the relationship between PIS-PK indicators and environmental factors in Palembang City in 2023. This study used an ecological study design with a quantitative approach. Secondary data were collected from the Palembang City Health Office and analyzed using Geographic Information System (GIS). As many as 7 community health centers (Puskesmas) achieved healthy family status, 33 Puskesmas were in the pre-healthy category, and 1 Puskesmas was in the unhealthy category; there was no directly proportional pattern between PIS-PK indicators and Puskesmas coverage; there was a directly proportional pattern between environmental health indicators and river water and PDAM (Regional Drinking Water Company) networks. The family health status in Palembang City in 2023 varied and was influenced by environmental factors, including accessibility to health facilities and clean water.
