Gnetum gnemon leaves contained flavonoids that had the potential to dissolve kidney stones by increasing urinary citrate excretion and diuresis. The low bioavailability of fractions required nanoparticle dosage forms as an option to increase the effectiveness of the delivery system. This study aimed to determine the percentage of nanosuspension entrapment efficiency used for in vivo tests and the effect of melinjo leaf fraction nanosuspension on calcium, creatinine, serum urea, and kidney histopathology levels in kidney stone rats. Nanosuspensions with three variations of chitosan 0.1%, 0.3%, and 0.75% were made using the ionic gelation method. The antrinefrolithiasis activity test was carried out in vivo on male rats induced by ethylene glycol and ammonium chloride. The positive control was given Batugin elixir, the negative control was given a placebo nanosuspension, and the test group was given a nanosuspension of ethanol and ethyl acetate fractions of melinjo leaves. The best formula was obtained from formula I with an average particle size of 191.77 nm, zeta potential -35.05 mV, PDI of 0.4018, and %EE of 96.7488% for ethanol fraction nanosuspension, and a particle size of 198.87 nm, zeta potential -35.15 mV, PDI of 0.3875, and %EE of 97.9526% for ethyl acetate fraction nanosuspension. The results showed a decrease in the average levels of creatinine, serum urea, and kidney calcium and an increase in urine pH in the melinjo leaf fraction nanosuspension test group. Duncan's statistical analysis showed no significant difference between the melinjo leaf fraction nanosuspension and the positive and normal groups (p>0.05). Ethyl acetate fraction nanosuspension was more effective in dissolving kidney stones than ethanol fraction nanosuspension based on histopathology results.
