The pre-analytical stage is a stage where many errors can occur, leading to reduced accuracy of blood pH examination. The Henderson-Hasselbalch formula in critically ill patients can provide vital information in evaluating patient conditions, especially in borderline pH values in Blood Gas Analysis examinations. Methods: This study was a cross-sectional study, conducted in the Medical Record and Laboratory Department of RSUP Dr. Mohammad Hoesin Palembang. The research subjects were all critically ill patients who underwent blood gas analysis in the ICU of RSUP Dr. Mohammad Hoesin Palembang during the study period that met the inclusion criteria and passed the exclusion criteria. Results: A total of 447 research samples were obtained from 155 research subjects. Based on the suitability test of acid-base disorders using the Kappa test, a Kappa value of 0.908 was obtained, indicating excellent strength. Based on the Pearson moment correlation test, a value of r > 0.8 was obtained, indicating very strong and statistically significant agreement. Conclusion: Based on the results of this study, it can be concluded that there is no significant difference between the pH values of blood gas analysis using the Nova pHox Ultra machine and the pH values of the Henderson-Hasselbalch formula. This study shows that the Henderson-Hasselbalch formula can be effectively used as an alternative in assessing acid-base balance in critically ill patients.
