PENGARUH EKSTRAK ETIL ASETAT BATANG KAYU KUNING (Arcangelisia flava) TERHADAP KADAR INTERLEUKIN-1 BETHA (IL-1β) DAN GAMBARAN HISTOPATOLOGI ANKLE JOINT PADA TIKUS PUTIH JANTAN GALUR WISTAR DENGAN HIPERURISEMIA
ABSTRAK PENGARUH EKSTRAK ETIL ASETAT BATANG KAYU KUNING (Arcangelisia flava) TERHADAP KADAR INTERLEUKIN-1 BETHA DAN GAMBARAN HISTOPATOLOGI ANKLE JOINT PADA TIKUS PUTIH JANTAN GALUR WISTAR DENGAN HIPERURISEMIA (Rona Hawa Kamilah, 07 Mei 2024, 125 halaman) Fakultas Kedokteran Universitas Sriwijaya Latar Belakang: Hiperurisemia adalah kondisi yang dicirikan oleh peningkatan kadar asam urat dalam darah. Prevalensinya telah mengalami peningkatan yang signifikan di seluruh dunia. Beberapa penelitian telah menunjukkan adanya korelasi antara kadar asam urat dalam darah dan pelepasan IL‐1β, yang memainkan peran penting dalam perkembangan hiperurisemia. Sinyal IL-1β saat ini dianggap sebagai pencetus awal dari peradangan akibat asam urat dan merangsang rekrutmen neutrofil ke lokasi peradangan. Allopurinol biasanya digunakan untuk mengelola hiperurisemia. Di sisi lain, peneliti sedang mengembangkan pengobatan alternatif dari bahan alami, seperti Arcangelisia flava (kayu kuning), yang telah terbukti memiliki efek antihiperurisemia. Hal ini mungkin disebabkan oleh kandungan metabolit sekunder seperti alkaloid, flavonoid, saponin, triterpenoid, dan quinon dalam tumbuhan tersebut. Penelitian ini bertujuan mengkaji lebih dalam pengaruh ekstrak etil asetat batang Arcangelisia flava terhadap penurunan interleukin-1β serum pada tikus putih Jantan galur wistar (Rattus norvegicus). Metode: Penelitian eksperimental secara in vivo dengan rancangan penelitian randomized prepost-test control group design menggunakan tikus putih Jantan (n=25) kondisi hiperurisemia terbagi dalam 5 kelompok: kontrol negatif (Na- CMC), kontrol positif (Allopurinol), dan 3 tingkat dosis ekstrak etil asetat batang Arcangelisia flava yaitu 100 mg/kgBB, 200 mg/kg BB, dan 400 mg/kgBB. Pengukuran kadar interleukin-1β dilakukan pada hari ke-0 dan ke-7 menggunakan metode ELISA , serta dilakukan pengambilan pulasan histopatologi ankle joint dengan pewarnaan HE. Hasil: Pemberian ekstrak etil asetat Arcangelesia flava selama 7 hari dapat menurunkan kadar interleukin-1β pada tikus. Uji Anova didapatkan nilai p=0,000 artinya terdapat perbedaan bermakna. Berdasarkan uji Post hoc dan rerata penurunan kadar interleukin-1β disimpulkan dosis III (400 mg/kg BB) merupakan dosis efektif dalam menurunkan kadar interleukin-1β. Hal ini juga dibuktikan dengan gaambaran histopatologi dosis III yang menunjukkan tidak adanya perdangan dan infitrasi neutrofil. Kesimpulan: Ekstrak etil asetat batang kayu kuning (Arcanglisia flava) dapat menurunkan kadar interleukin-1β pada tikus putih jantan galur wistar kondisi hiperurisemia Kata Kunci: Arcangelisia flava, antihiperurisemia, interleukin-1β, histopatologi ABSTRACT THE EFFECT OF ETHYL ACETATE EXTRACT FROM Arcangelisia flava ON IL-1BETHA LEVELS AND HISTOPATHOLOGICAL OVERVIEW OF ANKLE JOINT IN HYPERURICEMIC MALE WHITE WISTAR RATS (Rona Hawa Kamilah, 07 Mei 2024, 125 halaman) Faculty of Medicine Sriwijaya University Background: Hyperuricemia is a condition characterized by elevated levels of serum uric acid in the blood. Its prevalence has significantly increased worldwide. Several studies have indicated a correlation between serum uric acid levels and the release of IL-1β, which plays a crucial role in the development of hyperuricemia. IL-1β signaling is currently considered as the initiating event in uric acid-induced inflammation and stimulates the recruitment of neutrophils to the inflamed site. Allopurinol is commonly used to manage hyperuricemia. On the other hand, researchers are exploring alternative treatments from natural sources, such as Arcangelisia flava (yellow wood), which has been shown to have anti-hyperuricemic effects. This may be attributed to the presence of secondary metabolites such as alkaloids, flavonoids, saponins, triterpenoids, and quinones in the plant.This study aims to further investigate the effect of ethyl acetate extract of Arcangelesia flava stems on the reduction of serum uric acid in male Wistar rats (Rattus norvegicus) Methods: This study is an experimental in vivo research with a randomized prepost-test control group design using male white rats (n=25) with hyperuricemia conditions divided into 5 groups: negative control (Na-CMC), positive control (Allopurinol), and 3 levels of ethyl acetate extract doses from A. flava stems, namely 100 mg/kg BW, 200 mg/kg BW, and 400 mg/kg BW. Interleukin-1β levels were measured on days 0 and 7 using the ELISA method, and histopathological examination of the ankle joint was performed with HE staining. Results: The administration of ethyl acetate extract from Arcangelesia flava stems for 7 days resulted in a reduction in interleukin-1β levels in rats. An ANOVA test yielded a p-value of 0.000, indicating a significant difference. Based on post hoc tests and the mean reduction in interleukin-1β levels, it is concluded that the III dose (400 mg/kg BW) is the effective dose in reducing interleukin-1β levels. This is also supported by the histopathological findings of dose III, which showed no inflammation and neutrophil infiltration.. Conclusion: Yellow wood stem ethyl acetate extract (Arcanglisia flava) can reduce interleukin-1β levels in hyperuricemia conditioned wistar male white rats.
