Background: Thalassemia is a hereditary hemolytic disease that is an autosomal recessive, characterized by a disorder in hemoglobin synthesis. The management of thalassemia, particularly major thalassemia, includes regular blood transfusions and iron chelation therapy to control iron accumulation. The iron level in the body can be determined through a common examination, which is the serum ferritin level test. The aim of this study was to understand the relationship between adherence to iron chelation consumption and the type of iron chelation with serum ferritin levels in pediatric patients with major thalassemia. Methods: An observational analytic study with a cross-sectional design was conducted on 86 pediatric patients with major thalassemia. The sampling technique employed was consecutive sampling. Data were collected through interviews, filling out the Morisky Medication Adherence Scale-8 questionnaire, and analyzing patient medical records, specifically serum ferritin levels. Results: The results of this study indicate that the majority of major thalassemia pediatric patients, 41 patient (47.7%) are non-compliant in consuming iron chelation therapy, with deferiprone being the most commonly used type of iron chelator, namely 51 patients (59.3%). Most patients have ferritin levels >2500μg/L, namely 76 patients (76.7%). There is a significant relationship between adherence to iron chelation therapy and serum ferritin levels; however, there is no significant relationship between the type of iron chelator consumed and serum ferritin levels. Conclusion: The adherence to iron chelation therapy shows a significant relationship with serum ferritin levels in pediatric patients with thalassemia major, whereas the type of iron chelation does not demonstrate a significant correlation. This study emphasizes the importance of adherence to iron chelation therapy in managing iron accumulation in pediatric patients with thalassemia major.
