GAMBARAN KEPUASAN PENGOBATAN PADA PASIEN ARTRITIS REUMATOID (AR) DI RSUP DR. MOHAMMAD HOESIN PALEMBANG
Latar Belakang: Artritis Reumatoid (AR) merupakan penyakit inflamasi autoimun kronik yang menyebabkan peradangan dan kerusakan pada lapisan sendi sehingga terjadi nyeri kronis, instabilitas, dan deformitas. Tercapainya remisi merupakan tujuan utama pengobatan yang mesti dicapai pada setiap pasien AR. Salah satu faktor yang memengaruhi rendahnya tingkat remisi adalah kepuasan dalam pengobatan yang secara signifikan memengaruhi keputusan pengobatan dan berdampak pada hasil pengobatan Sampai saat ini, belum ada penelitian mengenai kepuasan pengobatan pada pasien AR di Indonesia. Metode: Penelitian deskriptif observasional dilakukan pada pasien AR di Poliklinik Reumatologi Ilmu Penyakit Dalam di RSUP Dr. Mohammad Hoesin Palembang. Data diambil melalui pengisian kuesioner dengan wawancara terpimpin pada 56 sampel berusia ≥ 18 tahun serta memenuhi kriteria inklusi dan ekslusi. Kepuasan Pengobatan dinilai menggunakan tool TSQM-9. Hasil: Dari 56 sampel didapatkan hasil terbanyak pada usia produktif (18 – 60 tahun) sebesar 85,7% dan jenis kelamin perempuan sebesar 89,3%. Didapati hasil paling banyak pada aktivitas penyakit sedang yaitu skoring DAS-28 ESR sebesar 42,9% dan DAS-28 CRP sebesar 46,4%. Mayoritas sampel menggunakan metroteksat tunggal sebagai pengobatan medikamentosa sebesar 64,3%. Berdasarkan skoring kuesioner pada setiap domain, didapati rerata tertinggi pada domain kenyamanan, yaitu sebesar 68,26. Rerata kepuasan global terapi tunggal csDMARD lebih tinggi yaitu 67,26, daripada rerata kepuasan global terapi kombinasi csDMARD sebesar 64,40. Kesimpulan: Mayoritas pasien AR berada pada rentang usia 18 – 60 tahun yang didominasi jenis kelamin perempuan. Sebagian besar pasien memiliki aktivitas penyakit sedang dan berat. rerata tertinggi pada subskor domain kenyamanan, diikuti domain kepuasan global, dan domain efektivitas. Kata Kunci: Kepuasan Pengobatan, Artritis Reumatoid Background: Rheumatoid Arthritis (RA) is a chronic autoimmune inflammatory disease that causes inflammation and damage to the joint lining, resulting in chronic pain, instability, and deformity. Achieving remission is the main goal of treatment that must be attained in each RA patient. One factor that influences the low remission rate is treatment satisfaction. Patient satisfaction significantly influences decisions regarding treatment and has an impact on treatment outcomes. Notably, there has been no research regarding treatment satisfaction in RA patients in Indonesia. Method: An observational descriptive study was conducted on RA patients at the Rheumatology Internal Medicine Polyclinic at RSUP Dr. Mohammad Hoesin Palembang. Data were collected by filling out a questionnaire with guided interviews from 56 patients aged ≥ 18 years who met the inclusion and exclusion criteria. Treatment satisfaction was measured using the TSQM-9 tool. Result: From 56 samples, the highest results were obtained in the productive age group (18 - 60 years) at 85.7%, and the female gender at 89.3%. The most common result was found in moderate disease activity, namely DAS-28 ESR scoring at 42.9%, and DAS-28 CRP at 46.4%. The majority of samples used methotrexate monotherapy as medical treatment at 64.3%. Based on the questionnaire scoring in each domain, the highest mean was found in the convenience domain at 68.26. The mean global satisfaction with csDMARD monotherapy was higher, namely 67.26, than the mean global satisfaction with csDMARD combination therapy at 64.40. Conclusion: The majority of AR patients are in the age range of 18 – 60 years and are dominated by the female gender. Most patients had moderate to severe disease activity. The mean global satisfaction with the TSQM tool is 66.46. The total average satisfaction score from the three domains is 65.61. Keywords: Treatment Satisfaction, Rheumatoid Arthritis
