Background: Hypertensive heart disease is a major cause of morbidity and mortality globally that tends to increase with age. The high prevalence of uncontrolled blood pressure leads to an increase in the occurrence of HHD, so the optimal use of antihypertensive drugs is essential in preventing and controlling HHD. This study aims to determine the drug utilization pattern of antihypertensive in elderly hypertensive heart disease patients at the polyclinic of RSUP Dr. Mohammad Hoesin Palembang Period of January - June 2023. Methods: This research is a descriptive study in the form of a drug utilization pattern study, with a sample of 157 patients who meet the inclusion and exclusion criteria. Data were collected by the total sampling technique using secondary data from patient medical records. Results: The characteristics of elderly HHD patients dominated by the age group 60-69 years (59,2%), male (51%), controlled blood pressure (68,2%), no complaints (35%), and have comorbidities (89,8%) with the most frequent comorbidity is CAD (25,5%). The most common therapeutic regimen was a combination of 2 antihypertensive drugs (42,7%), with the most frequent drug class combination being CCB + ARB (21,7%). The most preferred drug is candesartan (31,1%) with a dose of 8 mg (19,9%), where 97.3% of the drugs were in the correct dose. The highest frequency of antihypertensive administration was 1x/day (98,1%), with 88.8% of drugs being appropriate in terms of frequency of administration. The interaction between antihypertensive drugs was 100% synergistic. Conclusion: The drug utilization pattern of antihypertensive in elderly HHD patients is dominated by the use of 2 drug antihypertensive combination therapy regimen, with the most common drug class combination being CCB + ARB.
