Introduction: Respiratory problems are one of the main reason children are hospitalized. If it is not addressed quickly, they can pose a risk of death. Mechanical ventilation is used to help patients breathe when their respiratory system is not functioning properly. However, the use of mechanical ventilation must be done carefully and not excessively as it can lead to complications. Method: The study was conducted with an observational descriptive research design on pediatric patients with mechanical ventilation in the PICU of Mohammad Hoesin General Hospital, Palembang during the October–November period. Data were taken in a cross-sectional manner. Sampling was done by total sampling method. Results: The majority of pediatric patients were aged 13–18 years (35%), were male (52.5%), had good nutritional status (50%), belonged to the neurologic underlying disease group (42.5%), and had comorbidities (60%). The most used mode was SIMV-PC (80%) and almost all patients used limit pressure (97.5%). Patients who improved were 52.5% and those who died were 47.5%. Conclusion: Neurological problems was the most common disease group requiring ventilatory support in the PICU. Almost half of the study sample died. The highest mortality rate was found in patients with assist-control mode.
