Background: Lupus nephritis is a term that refers to serious kidney involvement from systemic lupus erythematosus (SLE), characterized by the presence of immune complex deposits in the anatomical components of the glomerulus. The diagnosis of lupus nephritis is generally made using a kidney biopsy. However, kidney biopsies cannot always be performed on children. This study aims to determine the relationship between renal manifestations and the stage of lupus nephritis based on renal histopathological features in children at RSUP Dr. Mohammad Hoesin Palembang. Method: This type of research is an observational analytic study with a cross-sectional design. The samples used were 35 children with lupus nephritis at RSUP Dr. Mohammad Hoesin for the period 2019-2022 who met the inclusion criteria. Sampling was carried out using the total sampling technique. Data collection was carried out by recording data from inpatient registers and medical records, then processed using univariate and bivariate analysis. Results: There is a significant relationship between the incidence of hypertension and the class of lupus nephritis based on histopathological features in children. Meanwhile, edema, nephrotic-range proteinuria, nephrotic syndrome, and AKI did not have a significant relationship with the class of lupus nephritis based on histopathological features in children. The hematuria variable cannot be analyzed statistically. Conclusion: There is a significant relationship between hypertension and lupus nephritis class based on histopathological features in children. There was no relationship between edema, nephrotic-range proteinuria, nephrotic syndrome, and AKI on the class of lupus nephritis based on histopathological features in children. The relationship between hematuria and the class of pediatric lupus nephritis could not be analyzed statistically. Keywords: The class of lupus nephritis, renal manifestations
