The high risk in the oil and gas industry demands an effective Occupational Health and Safety Management System (SMK3). Despite technical safety improvements, accidents still occur, and human factors become dominant. PT X, as a contractor for the Oil and Gas Mining Business Entity (KKKS), has implemented SMK3 and focused on a safety culture. Safety culture at PT X is a primary focus with leadership programs, including the Supervisory HSE Expectation Card, also known as the Supervisory Checklist (SCL). This program involves line management in daily activities such as safety talks, safety observations, and Site Self Audits. Life Saving Rules (LSR) are integrated to emphasize life safety. Regular safety meetings and HSE campaigns have also had a positive impact, resulting in a decrease in accidents over the past three years. Despite positive progress, the maturity level of the safety culture in 2022 reached 2.93, slightly below the Management's target of 3 (independent). Therefore, this research will explore employees' perceptions of leadership and management commitment, OHS objectives, targets, and work program, OHS regulations and procedures, OHS information and communication, as well as training, knowledge, and employee competence.
