BPS reported that in 2020, diarrhea cases among under-five children in Palembang City were the highest in South Sumatra. The risks of diarrhea among under-five children are related to the environment and behavior in the community. This study aimed to analyze the correlation between behavioral and environmental factors with the incidence of diarrhea in under-five children and determine priority areas for handling diarrhea among under-five children in Palembang City in 2020-2022. This type of research is quantitative analytic with ecological study design. The sample used was the entire work area of 41 public health centers (PHCs) in Palembang City for 3 years with a total of 123 samples. This study used secondary data from the Palembang City Health Office. The data analysis used were Rank Spearman correlation and spatial analysis using the Quantum GIS application scoring technique. The results of the correlation test showed a significant relationship with a negative direction between the variables of the ODF village (r=-0.203 and p=0.024), health house (r=-0.308 and p=0.001), access to improved latrines (r=-0.295 and p=0.001) and access to improved drinking water (r=-0.313 and p=0.000) with the incidence of diarrhea in under-five children. The results of the spatial analysis show that there are 5 PHCs with very high diarrhea vulnerability in 2020, 1 PHC in 2021, and 6 PHCs in 2022. This study concludes that there is a significant relationship between ODF villages, health houses, access to improved latrines, and access to improved drinking water with the incidence of diarrhea in children under five, and there are 6 priority public health centers for handling diarrhea in children under five in Palembang City, including 7 Ulu, Kertapati, Keramasan, Karya Jaya, Pembina, and Nagaswidak.
