The development of a three-tier diagnostic test instrument to identify students' misconceptions about human movement system material in high school aims to determine the validity and reliability of the three-tier diagnostic test of human movement system material in high school students and to determine the profile of misconceptions among high school students in Lubuklinggau City. This study uses the development model of Treagust which has three main steps, namely determining the content of the material, seeking information on misconceptions and developing a three-tier diagnostic test instrument from the results of students' misconception information. Human movement system material developed using Sherwood's book. The three tier diagnostic test instrument is arranged in the form of a reasoned multiple choice equipped with a CRI. The results of the three tier instrument were validated by five validators using CVR and obtained a value of 1 which indicates that the three tier diagnostic test instrument is valid and relevant for detecting misconceptions. The results of the instrument reliability test using the KR-20 formula were 0.86 in the choice of questions categorized as high and 0.77 in the choice of reasons categorized as high. From the results of the misconception test on each sub- matter as follows: skeleton 47.35%, joints 47.33%, muscles 41.75%, and abnormalities in the human movement system 35.00%
