Throughout the observations made by the author at the PMB Clinic Chaya Mother there are still some shortcomings, especially in the data collection process that still uses handwriting in the notebook. This results in frequent errors in recording and the risk of damage and loss of books, which make data security cannot be guaranteed. In addition, PMB Clinic Chaya Mother does not have a website that can be used by the community or patients to get information about the list of doctors, list of facilities and list of medical services, as well as patients when they want to make a visit or examination at the clinic, must come and go back and forth to take the number queue, which is very time and energy. Through applications built by the writer, it is sure to help patients submit a visit form from home so that it can save time and energy. It also makes it easier for the clinic in managing doctor data, facility data and medical service data, as well as facilitating the preparation and printing of reports for the leadership. From the research results and test results the authors conclude that the application that was built was in accordance with the research objectives and in accordance with the needs of PMB Chaya Mother's clinical management.
