Kelurahan Plaju Darat Office, in its work activities, really needs data on the medical records of residents and patients in mapping the health status of residents in a certain period of time, for example how many patients have contracted Covid in one month in the Kelurahan Plaju Darat area, so village officials will come to Health Center and request a report from them, which will take a long time and process and is very time-consuming and energy-consuming for employees at the Kelurahan Plaju Darat office. This is because there is no integrated system between the Puskesmas and the Kelurahan Plaju Darat Office. This research aims to create and develop a system in Kelurahan Plaju Darat so that it can coordinate and exchange data regarding residents' health so as to facilitate the process of making reports. From the results of the research and the results of tests carried out by the author, it can be concluded that the application is in accordance with the research objectives and according to the needs of the Kelurahan Plaju Darat Office.
