Latar belakang: Pusat kesehatan masyarakat (Puskesmas) adalah unit pelaksana teknis dinas kesehatan kabupaten/kota yang menyelenggarakan fungsi upaya kesehatan masyarakat (UKM) dan upaya kesehatan perseorangan (UKP). Puskesmas Makrayu merupakan salah satu puskesmas dengan cakupan luas wilayah dan cakupan masyarakat terbesar di wilayah Kota Palembang. Penelitian ini bertujuan untuk menilai pelaksanaan Sistem Informasi Kesehatan (SIK) pada program KIA di Puskesmas Makrayu. Metode: Penelitian ini adalah penelitian mixed method research. Pengambilan data dalam penelitian ini menggunakan formulir HMN versi 4, wawancara, dan observasi. Informan penelitian ini adalah Kepala Puskesmas Makrayu, Kepala Tata Usaha Puskesmas Makrayu, Penanggung Jawab Program KIA di Puskesmas Makrayu, dan Pelaksana Program KIA di Puskesmas Makrayu. Hasil: Penilaian komponen sumber daya SIK sebesar 98,4%, komponen indikator SIK sebesar 100%, komponen sumber data SIK sebesar 100%, komponen manajemen data SIK sebesar 84%, komponen produk informasi SIK sebesar 96%, dan komponen diseminasi dan penggunaan data sebesar 98%. Kesimpulan: Keenam aspek SIK pada program KIA di Puskesmas Makrayu sudah berada pada kategori sangat adekuat, namun masih terdapat beberapa kendala seperti aplikasi yang digunakan terlalu banyak dan tidak terhubung satu sama lain, jaringan internet yang lambat, tidak adanya anggaran dana untuk pencatatan dan pelporan kegiatan KIA, kurangnya jumlah komputer pada poli KIA, tidak tersedianya gudang penyimpanan data dan informasi pada bagian tata usaha,serta tidak sesuainya data target capaian Dinas Kesehatan dengan data asli di lapangan. Kata kunci: Puskesmas, KIA, SIK, HMN.   Background: Pusat Kesehatan Masyarakat (Puskesmas) is a technical implementation unit for the district/city health department that carries out the functions of community health program (UKM) and individual health program (UKP). Makrayu Health Center is one of the health centers with the widest area coverage and the largest community coverage in the Palembang City area. This study aims to assess the implementation of the Health Information System (SIK) in the MCH program at the Makrayu Health Center. Methods: This research is a mixed method research. Data collection in this study used the HMN version 4 form, interviews, and observations. The informants of this study were the Head of the Makrayu Health Center, the Head of Administration of the Makrayu Health Center, the person in charge of the MCH Program at the Makrayu Health Center, and the MCH Program Executor at the Makrayu Health Center. Result: Assessment of HIS resources component is 98,4%, HIS indicator component is 100%, HIS data sources component is 100%, HIS data management component is 84%, HIS information product component is 96%, and HIS dissemination and information usage components is 98%. Conclusion:The six aspects of the HIS in the MCH program at the Makrayu Health Center are already in the very adequate category. However, there are still a number of constraint, such as too many applications used in the MCH program and not connected to each other, the internet network is slow, funding is not budgeted for recording and reporting MCH activities, lack of computers in the MCH poly, unavailability of warehouses for storing data and information in the administration section, and the incompatibility of the target data for the achievements from department of health with the original data in the field.
