PT Tambang Bukit Tambi is a subsidiary of PT Nan Riang which is located in Padang Kelpo Village, Maro Sebu Ulu District, Batanghari Regency, Jambi Province. In this study, analysis of potential hazards and risk control was carried out using the Hazard Identification Risk Assessment and Risk Control (HIRARC) method. With this HIRARC method, we can identify the hazards and the hazard character of the work that is done as routine or non-routine. Work safety risks at the loading stage, namely, the operator collides with the body unit, sprains, slips, gets caught in the cabin door, the unit collapses, the Excavator unit overturns, collides with the Dozer unit, the Dump Truck unit hits the Excavator unit, the Dump Truck unit hits a small vehicle unit, is crushed by material load and the risk of collision between the Excavator bucket and the DumpTruck body. The work safety risks at the hauling stage are, the DumpTruck unit crashes into another unit, collisions between units, the Dump Truck unit crashes into the embankment, the Dump Truck unit overturns, and the risk of the Dump Truck unit being burned. Work safety risks at the dumping stage are collisions between units, the Dump Truck unit slips, the Dump Truck unit hits an embankment, the Dump Truck unit hits another unit, the Dump Truck unit overturns when maneuvering, and the risk of the Truck unit falling and tipping over when dumping. In mining activities, the company recorded 3 minor accidents. The company has received a zero accident award in 2019. It has never been sanctioned against PT Tambang Bukit Tambi due to violations of the implementation of K3 by the local government or the Ministry of Energy and Mineral Resources. In managing K3 the company can be said to be successful in terms of controlling work accidents that can cause fatalities and failing in terms of controlling dust and road surface quality. Things that support the success of K3 management is the activeness of the Health and Safety department in K3 supervision, on the other hand the cause of failure is the human fatigue factor in production operation operators.
