Diarrhoea is one of the most common infectious diseases in children under five, which can be caused by several risk factors, including the agent, the host and the environment. The purpose of this study was to determine the risk factors for the incidence of diarrhea in toddlers aged 12 - 59 months in the watershed and non- watershed area of Kelurahan 15 Ulu, Jakabaring District, Palembang City. The design of this study was analytic observation with a case control. Sampling technique was purposive sampling so that 132 respondents were obtained, each 66 mothers of toddlers in the watershed and not watershed (22 cases, 44 controls). Data were analyzed using Chi Square and Fisher Exact tests, and multivariate with multiple logistic regression prediction model. The results showed that most respondents in the watershed area had an unqualified clean water source (p=0.002), unqualified drinking water source (p=0.003), unhealthy latrine conditions (p=0.001), and toddlers did not have a history of exclusive breastfeeding (p=0.000) so that there was a relationship between the four variables with the incidence of diarrhoea in toddlers in the watershed area. While the majority of toddlers in non-watershed areas do not have a history of exclusive breastfeeding so there is a relationship between exclusive breastfeeding history with the incidence of diarrhoea in toodlers in non-watershed areas. The probability of diarrhea in each area is 83%. The risk factor most related to the occurrence of diarrhea in the baby in the watershed area is the source of drinking water (OR = 14,971), while in the non-watershed areas is the history of exclusive breast milk (or = 32,325). The conclusion of this research is that the risk factor for diarrhea in the watershed area is most dominantly from environmental factors, while in the non-watershed area it is from the balita factor so that special inspections are necessary for basic sanitary means of the environment in the watershed area as well as advanced socialization on the importance of baby body immunity in the prevention of diarrhoea in the non-watershed areas.
